After you finish completing your paperwork, email your documents to us along with a copy of your Dental Insurance information to [email protected]. Don't have a physical dental insurance card? No worries, we accept digital copies as well. Emailing everything to us first will allow us to attend to your medical needs more quickly than completing them on your arrival.
